TO EXECUTE A ~EDICARE PARTICIPATING
SUPPLIER AGREEMENT
BE IT RESOLVED, that:
The aayo= is he=e and hereby authorized and directed to enter into
the annexed mEDICARE PARTICIPATING PHYSICIAN OR SUPPLIER AGREEMENT ~
with Kttsap Physicians Service to allow payment for ambulance service
under the medicare program°
