The above-reference proposal was reviewed by this department relative to the International Fire Code [I.F.C.], 2003 Edition, and the following constitutes this department's findings and determinations based upon the plans of record submitted dated 03/21/05. Findings& Determinations: 1. The proposal was reviewed as a 2-story with daylight basement Group B occupancy with a total of 18,850 square feet of fire area [largest single separated floor area = 6,020 square feet] with a Type VB construction classification[s]; and, 2. Addressing for the proposal shall be consistent with the Municipal Code for size, and be in a position as to be plainly visible and legible from the street or road fronting the property. Said numbers shall contrast with their background [I.F.C. Section 505], and; a.Tennant identification shall be consistent with Section 408.11.2 3. Key box access to or within the subject structure for emergency services delivery is required to be installed at the main entry and at the northeast entry location and shall be a SUPRA Suprasafe Model 2HSR-TS (with tamper switch wired to building security), to contain key(s) to gain necessary access to the structure in its entirety, as required by the Chief of this department [I.F.C. 506]; and, 4. Road access shall be provided for this proposal is found to be consistent with Section 503 and Appendix D fire apparatus access road design standards. 5. Access to building openings is designed consistent with the I.F.C. Section 504 which requires an approved access walkway leading from fire apparatus access road(s)to exterior openings that are required by the Fire Code or Building Code,and; 6. An automatic fire suppression system(sprinklers) is not required under I.F.C. Section 903, 7. Fire extinguisher sizing and placement shall meet or exceed IFC Section 906 and NFPA Standard 10, which normally requires a 2-A:10-B:C fire extinguisher at the exit(s)and; 9. Jurisdiction for fire flow and fire hydrant review for this proposal is derived from the requirements of the City of Port Townsend Engineering Design Standards, Section 903.2, and,Appendix B and C of the I. F. C., and applying the maximum spacing rules for mixed use, multi-family, and commercial areas as defined by this project [see attached concentric fire hydrant spacing map]: 10. The project is not eligible for the area separation fire flow allowance of ITC Section B104. Portions of buildings separated by fire walls can not have openings. This proposal has fire doors that constitute openings prohibited by the Code, and ; 11. Fire flow for this proposal is based upon a total fire area of 18,850 square feet. The construction classification proposed is Type V-B. Fire flow requirement for this proposal is 3,750 gallons per minute sustainable for two [2] hours, from four[4] fire hydrants a. H146 at SR20 and Howard Street is the existing hydrant and meets the proximity requirement of the City's Engineering Design Standard [Chapter 2 "water", Section 1, which requires an existing fire hydrant within 250 feet of a project site, and; b. Contractors working on this project are required to have a Labor& Industries contractor's registration number and a City business license. Failure to provide proof of this documentation prior to work may result in job shut down while this is accomplished. 2. Temporary erosion and sediment control (TESC) measures shall be installed on-site and inspected prior to beginning construction; call 385-2294. Measures shall include installation of silt fencing and graveled construction entrance (see attached details). Adjacent rights-of-way shall be kept free of dirt debris. Soils exposed during construction shall be temporarily stabilized with mulching,plastic sheeting, etc. Soils shall be permanently stabilized with seeding, plantings,sodding, etc. once construction is complete. Applicant is responsible for protection of adjacent properties. 3. All elements of engineering including nailing, holdowns, sheathing, and alternate braced wall panels (ABWP) require inspection prior to cover. 4. Owner or owner's agent shall review and oversee correction of any and all deficiencies noted by required inspections. 5. Parking for approximately 60 cars would be provided, accessed via Howard Street. The proposed building will be subject to Commercial Design Review. The completed project will include landscaping and will meet the provisions of PTMC Chapter 19.06, Tree Conservation. Interim street improvements will be provided pending the potential establishment of a Howard Street Local Improvement District (LID). Infrastructure costs for permanent improvements will be assured by bonding or similar approved means of guarantee. Prior SEPA review for the construction of three buildings on this site was completed on June 2, 2004. This SEPA addendum is required to address a modified proposal that will involve the construction of a singular larger building and increased site grading. The Jefferson County Assessor's parcel number for the parent property is 001 094 003 lying within Section 10, Township 30 North, Range 1 West, WM. Threshold Determination: A Mitigated Determination of Non-Significance was issued on June 2°a, 2004 and is hereby incorporated by reference. The lead agency for this proposal has determined that, with proper mitigation, the revised project does not have a probable significant adverse impact on the environment. An environmental impact statement(EIS) is not required under RCW 43.21C.030(2)(c). This decision was made after review of amendments to the environmental checklist and other information on file with the lead agency. This information is available to the public upon request. This-SEPA Addendum is issued under WAC 197-11-600(4)(c) and 197-11-625; A SEPA Addendum is appropriate when a project has been revised and the changes do not result in any new significant environmental impacts. ENVIRONMENTAL REVIEW BACKGROUND Prior SEPA review for the construction of three buildings on this site was completed on June 2, 2004. This SEPA addendum is required to address a modified proposal that will involve the construction of a singular, larger building and increased site grading. Revised and/or additional mitigating measures that address these changes have been required. I. Revised Proposal Project Description The revised project will consist of one building with approximately 17,000 square feet of floor area as compared to the total of 15,000 square feet of area associated with the three buildings originally proposed for the site. The proposed building will be located fronting Sims Way (SR 20) and will result in more extensive site grading than originally evaluated. Approximately 2,040 cubic yards of material will be removed with approximately 460 cubic yards being retained on site as fill. The next export of material will be 1,580 cubic yards which is proposed to be stored on the adjacent Lot 2 of the Wolcott Short Plat. The proposed use of the building is to accommodate Re-Max Realty's offices. Commercial space will also be available fronting Sims Way. No proposed tenants have been identified. II. Analysis The following analysis supplements and amends that contained in the MDNS issued on June 2, 2004. The proposed modifications to the proposal will effect the environment in the following areas: a. Earth. The modified proposal increases the amount of excavation on-site and results in a net export of materials to an adjacent property (Lot 2, Wolcott Short Plat). Expanded grading will require a revised grading plan. To reduce impacts to traffic, grading should be accomplished from the Howard Street Right-of-way (ROW). A Traffic Control Plan should be prepared to address the use of portions of the Howard Street ROW to facilitate site grading. Coordination with the Washington State Department of Transportation (WDOT) will be required to address potential impacts from the proposed excavation on the use of SR 20. In addition, the off-site placement of materials on Lot 2, Wolcott Short Plat should not harm significant trees, unless such action is permitted through the approval of a tree conservation plan. b. Water Runoff. The modified proposal will require revision to the submitted Stormwater Plan to address site ( � plan alterations. If infiltration is proposed, testing that demonstrates soil suitability should be included within the revised Plan. In addition, the off-site area(Lot 2, Wolcott Short Plat) that ffII has been identified as the receiving area for excavated materials will need to be included in the b� revised Temporary Erosion and Sedimentation Control (TESC)Plan. C. Plants. The modified proposal will remove trees that were previously identified for retention. The removal of trees warrants review consistent with the standards and requirements of Chapter 19.06 PTMC (Tree Conservation). The Tree Conservation Plan should include the off-site receiving area for excavated materials. In addition, every effort should be made to avoid, preserve or relocate existing street trees found within the Howard Street ROW. d. Transportation. The modified proposal indicates that site ingress/egress will be facilitated from Howard Street. Access from SR 20 and or the adjacent Hilltop Tavern site is no longer anticipated at this time. However, the Applicant wishes to preserve alternative access options for future use. Therefore, the access alternatives established in the MDNS remain appropriate. The Applicant will be responsible for the cost and establishment of transportation improvements directly associated with the subject proposal that are over and above those guaranteed by the Wolcott Short Plat. e. Utilities. Currently, water and sewer service does not adequately serve the subject lot. However, subsequent to the issuance of the MDNS, the Wolcott Short Plat received Final Plat Approval. Utility improvements have been guaranteed by the Wolcott Short Plat. Expenditure of the guaranteed funds should occur concurrent with site development. To mitigate potential water and soil erosion hazards, a clearing and grading permit application together with final engineered plans for clearing, grading and temporary erosion and sediment controls (TESC) must be submitted to the BCD Department. A separate clearing and grading permit will only be required if grading will occur prior to the issuance of a building permit. The Clearing and Grading Plan and TESC shall include a detailed narrative explaining the proposed method and approach to be used to grade the site. The applicant shall contact the Washington State Department of Transportation and obtain all necessary permits and approvals-prior to any use of the SR 20 ROW for site grading These plans shall be reviewed by BCD with the Public Works Department for compliance with the Port Townsend Engineering Design Standards and the Department of Ecology Stormwater Management Manual (DOE SWMM). Temporary erosion and sediment control measures shall be installed as specified by the approved plans before beginning clearing and grading activities and shall be maintained until the city Public Works Department deems them unnecessary. If removal of soils from the site is required, the applicant shall first notify the Director of Building and Community Development(BCD) of the destination of the soil. If the destination site is within the City of Port Townsend, the BCD Director must approve the location of the site and the necessary erosion control measures associated with the soil relocation. 2. If earth disturbance occurs in drier months, water shall be applied as a dust suppressant. 3. A copy of these mitigation measures shall be given to andread by the project contractor and all sub-contractors prior to beginning construction, and a list of these mitigation measures shall be prominently posted at the site in a waterproof container or bag. To mitigate potential impacts to transportation and utilities, the applicant shall prepare an agreement, in a form acceptable to the City Attorney, stipulating the means of providing assurance for costs associated with the future construction of required street, utility and frontage improvements, and further providing that, in addition to other remedies, failure to timely complete the improvements on direction of the City allows the City to revoke occupancy permits and enforce a failure as a violation of the zoning code. Upon determining that the improvements will be required, the City shall notify the applicant in writing and inform him that appropriate development permit applications and materials must be submitted to the City within ninety (90) days from the date of the notice. The applicant shall be allowed one-hundred and eighty (180) days from the date of development permit approval to construct the improvements. Should the applicant fail to construct the improvements within the allotted time period, the City shall revoke the occupancy permit and initiate an enforcement action. The required construction of these improvements shall be temporarily suspended pending the evaluation of the formation of a Local Improvement District(LID). The period of the temporary suspension of required improvements shall not exceed three (3) years from the date of the initial agreement unless extended by agreement of both the City and the applicant. ,All proposed means of assurance must be irrevocable until extinguished by mutual agreement of the City and the applicant and shall run with the land. A revised site plan shall.be submitted that illustrates the relocation of the proposed buildings and all proposed structures such that they do not encroach on, or intrude within, the easements established for Lot 1 of the Wolcott Short Plat(LUP96-00068). The areas with these easement areas shall be reserved for temporary trail and landscape use. 8. To mitigate transportation impacts to the City's streets and to ensure compliance with City Engineering Design Standards and city codes related to site layout, frontage requirements, and utility service, the following plans must be prepared and submitted with a completed application for a Street and Utility Development Permit for work proposed in city rights- of-way." The applicant is strongly encouraged to consult with the Public Works Department via a Technical Conference prior to commencing with engineering design. The required plans should be submitted with the completed building permit application and include: a. A parking plan including the subject building uses,parking stall dimensions and numbers, ADA parking, the bicycle rack location, provisions for pedestrian access through the parking lot, and provisions for landscape screening meeting the requirements of PTMC Chapter 17.72 and PTMC 17.20. In addition, the parking plan shall include all internal circulation driveways, the joint use parking area and the common access driveway. To mitigate impacts to circulation the joint use e parking area shall be paved and striped in compliance with Section 17.72 PTMC. Internal circulation as proposed in the parking plan shall accommodate the turning radius of dial-a-ride vans and emergency vehicles. To mitigate impacts to bicycle parking'the final site and parking plan shall show a bicycle parking area. Bicycle parking should be located as close as possible to customer and employee entrances. Covered bicycle parking is strongly recommended. The final parking plan must be approved by the BCD Director in consultation with the Public Works and Fire Departments prior to issuance of the building permit. Preparation of the parking plan should be coordinated with the landscaping plan. b. Plans for the temporary street frontage improvements including the locations of driveway(s), six foot(6') wide soft-surface trails and drainage facilities. In order to mitigate impacts to transportation associated with site access and parking the applicant shall comply in full with ONE of the following mitigation approaches: i. Engineered plans for SR 20 improvements as required by WASHDOT. These plans shall show the location of the proposed"C"curbing on the mainline (SR 20) between the eastbound left turns at the proposed access SEPA Addendum LUP 04-020 February 16. 2005 Page 7 of 11 and the location of signage (right turn only) and curbing (along the Hilltop Tavern frontage)that defines the proposed joint use access driveway. The intent of defining the entrance is to prevent left turn motions from the driveway. The design of the curbing shall prevent circumvention of this restriction by encouraging traffic to exit via Howard Street rather than from other points on the Hilltop property. These plans shall be approved by the City Engineer and WASHDOT and all work shall be completed prior to the issuance of a Certificate of Occupancy. OR ii. Engineered plans for consolidated access and SR 20 improvements for the subject property and the adjacent Hilltop Tavern property. These plans shall show a common, consolidated access driveway that serves both the Hilltop Tavern and the subject project from Sims Way (SR 20). This common access driveway shall be located no closer than three-hundred and thirty-three (333') from Howard Street. The driveway shall be paved, as shall the internal roadway connection to the parking areas serving the subject project. Stormwater runoff from all proposed off-site impervious surfaces shall be accommodated consistent with City Standards. The engineered plans and a signed letter of agreement from the adjacent property owner shall be submitted to both the City and WASHDOT for their review and (if found consistent with adopted standards) approval. OR iii. A revised parking plan (see Mitigation Measure 8(a) above)'that eliminates the need for a common access driveway on Sims Way(SR 20). The revised parking plan shall establish a reserve parking area on the subject property. The reserve parking area shall be accessed from the Howard Street entrance to the property and shall be sized to accommodate no less than twelve (12) full size parking spaces. The reserve parking area shall not be required to be improved until such time (if ever)that building uses require additional spaces(consistent with the requirements of Table 17.72.080 PTMC). Access to proposed building "C" shall be provided from the Howard Street access driveway. This access shall be paved and shall be segregated and physically separated from the Hill Top Tavern's adjacent unimproved parking area. Egress to Sims Way(SR 20) from the project shall not be available though the Hill Top Tavern parking area. The applicant acknowledges that the reserve area may be required to be improved prior to use or change of use of the proposed buildings (consistent with Section 17.72.060 PTMC). d. Engineered plans for tee water service and sewer service for the project including provisions for any necessary fire flow improvements and/or hydrant(s). The Fire Department must establish fire flow requirements for the project. A final engineered stormwater drainage plan including construction drawings complying with the requirements of the Puget Sound Stormwater Management Manual and the Port Townsend Engineering Design standards must be submitted to BCD and approved by the Public Works Department prior to the issuance of the building permit. If on-site infiltration is proposed the results of field testing confirming soil suitability shall be included in the plan The plan shall include the detention, treatment and release of stormwater from the joint building, parking area.. and the off-site receiving area used to store excavated materials from the subject lot f. A request(If applicable. See 8(c) above) for Joint Use Parking approval consistent with the requirements of Section 17.72.140 PTMC. 9. An application for an Access Connection Permit for the joint access driveway shall be submitted to WASHDOT(If applicable. See 8(c) above). The application shall be approved prior to any construction activities associated with the Joint use parking area or it's related access points. 10. To mitigate aesthetic impacts of the proposal a landscaping plan shall be submitted to BCD for review and approval. The landscaping plan shall comply with the requirements of PTMC 17.72 and PTMC 17.20. The plan should include native and drought tolerant plant species to conserve irrigation water and provisions for irrigation until plants are established. A list of native and drought tolerant plants is available at the BCD office. The final plan shall be drawn to scale showing all property lines and the following information: a. Any proposed or existing physical elements, such as fencing, berms, walls, buildings, curbs, and lighting, that affect the overall landscape; b. The location of all proposed plants, number of plants, and the species are to be shown by botanical and/or common name. A planting schedule shall be included that shows all plants, quantities, sizes at planting and spacing. A Tree Conservation Plan prepared consistent with the requirements Chanter 19.06 PTMC shall be submitted for the review and permit. shall be approved prior to the issuance of a buildin 11. To mitigate potential lighting impacts to adjacent properties and street rights-of-way, the applicant will submit an exterior lighting plan (including any parking lot light poles) for review and approval by the BCD Director. Illumination intensity shall be kept to a minimum, and all lighting fixtures shall be shielded, and pointed downward, including lighting for signs. The lighting shall include proposed average illumination levels and be designed so as not to cause glare or light spillage to adjacent streets or other off-site SEPA Addendum LUP 04-020 February 16. 2005 Page 9 of 11 properties. Revisions to the lighting may be required if the Director of BCD determines that installed lighting fails to meet the above performance standards. To reduce off--site glare impacts, pole mounted light fixtures shall not exceed 15 feet in height. Additionally, no mercury vapor or halogen lights are permitted in any exterior or parking lot light fixture. The use of high or low pressure sodium type fixtures is encouraged. DURING CONSTRUCTION 12. New power and telephone lines must be placed underground where feasible, as determined by the Public Works Director. 13. To avoid environmental health hazards during construction, emergency spill and cleanup procedures shall be explained to all workers on the site and a point of contact will be designated. 14. All construction contracts shall specify that all diesel, gasoline and air-powered equipment must be properly muffled or silenced. 15. To mitigate potential impacts to archeological resources that may be discovered during excavation, work should be stopped immediately if such materials are discovered and the State Historic Preservation Officer contacted immediately. Work may not resume until approval is obtained from the Director of Building and Community Development. 16. To mitigate the impact of construction noise outside construction activities shall be limited to 7:00 a.m. to 6:00 p.m.. The performance security shall consist of a performance bond in a form acceptable to the City Attorney and in an amount acceptable to the Director. Cash deposited in an escrow account may also be accepted by the city. All required plantings shall be installed within six months of approving the performance security. 19. All temporary improvements as specified by the Street and Utility Development Permit shall be installed, inspected and approved by the Public Works Department prior to the final certificate of occupancy. These improvements shall include but are not limited to water and sewer connections, driveways, trails and stormwater facilities. 20. The proposed stormwater treatment facilities and drainage system located on-site will be privately operated and maintained by the property owner. As a condition of approval an operation and maintenance plan and covenants ensuring perpetual operation and maintenance of the facilities must be prepared, approved by Public Works, and the covenants recorded with the Jefferson County Auditor's Office. 21. The developer will construct all pedestrian and parking facilities specified in the approved parking and site plans prior to issuance of the final occupancy permit. 22. Fourteen staff are on-site all day. In addition, at any given time there are 15-16 clinical staff working who will need a place for between 1-2 hours at a time. There are 3 office staff and 1 office manager. The 2 staff can share an office, while the manager has a private office. Support devices, such as the copier and medical records should be located here. There is one triage nurse who spends 8-hour days on the phone, receiving questions and coordinating staff and patient concerns. They need dedicated space, though not an enclosed office. This needs to be large enough to accommodate a person being trained. There will be one quality person full time. We have 5 other administrators and 1 administrative assistant. These staff need to meet with their staff one on one, meet with community members and/or vendors. Four need to be close to where the Clinical staff with private offices are, and the other two can be located elsewhere. We have 2 clinical specialists, one hospice RN and one Physical therapist. They can use shared space, but each need an additional chair by their desk to accommodate staff training needs. They need to be next to the area where the RN's space. One large (50 people) conference room is necessary. This will be used for group meetings of the different disciplines and the interdisciplinary team meetings that meets l- 2 x weekly. The second small conference room needs to accommodate 5-6 staff and would also double as a lunch room or a training room. Social work needs a shared office for the manager and the hospice volunteer coordinator. Two other social workers are part time. These four staff should be near each other and in close proximity to the hospice director. We will need a small space for reference material and patient education material. This will be near the kitchen alcove.
