10. Erosion and sediment control measures and construction entrance must be installed and inspected prior to
construction. Silt fence, construction entrance, etc. must be maintained throughout construction.
20. Pave an asphalt driveway apron from the edge of street, 10 ft. deep. Maximum driveway width in residential is 20
ft. Saw cut edge of street and tack and seal well. Call for inspection of driveway prep prior to paving. Noti$'City
when paving is scheduled.
25. Follow Richard Hefley's Arborist Report datedill5l08. Including the "Pre-Construction Recommendations",
"Construction Recommendations", and "Post-Construction Recommendations"
30. Connect to existing sewer in Root Street. Install a backflow preventer and a cleanout. Call for inspection before
covering.
40. Submit an as-built drawing of side sewer connection at time of inspection on form provided with permit.
50. May connect to existing water in Root Street. City installs meter. When all fees are paid, call 379-4434 for meter
installation. Allow l-32weeks. Property owner connects from the meter to the building.
60. Install on-site stormwater management per perk tests, Engineering Design Standards and DOE 2005 Stotmwater
Management Manual and Engineering Design Standards. Inspection required before covering. Attached is Detail
SW-2 from the City's Engineering Design Standards and page 3-7 from the DOE Stormwater Management Manual
on Hydrologic Analysis and Flow Control BMPs. Use these standards for on-site stormwater management. As an
alternative, included is some information on Infiltrator systems. These are to be sized for the capacity needed.
70. All areas of work must be restored. If any damage occurs to existing infrastructure during construction, damage
must be repaired, inspected and approved by the City.
80. All conditions on this permit must be met, completed, inspected and approved by the City before this permit is
signed off.
